Plano Speech Language Pathologist

Responsibilities and Duties Assess patient needs and develop individualized treatment plans based on physician orders and interdisciplinary team input. Provide direct speech therapy services to patients in their homes or assisted living facilities within a defined geographic area. Ensure timely documentation and compliance with all Paradigm HomeCare policies and procedures. Collaborate with physicians, nurses, and the care team to support optimal patient outcomes.
