Primary Care Physician (PCP) Location: Baltimore, MD Job Type: Full-Time New graduates are welcome to apply if actively licensed Monday through Friday 8:00 AM - 5:00 PM Outpatient Center Job Summary & Responsibilities: Weare seeking compassionate and patient-focused Primary Care Physicians (PCPs) to join a leading value-based care organization dedicated to improving healthcare outcomes for seniors. The PCP serves as a leader within a multidisciplinary, team-based care model focused on providing high-quality, coordinated, and personalized care for geriatric patients. The physician will evaluate and treat patients in an outpatient setting while collaborating closely with advanced practice providers, pharmacists, care associates, behavioral health specialists, referral coordinators, and other healthcare professionals. This model allows providers to spend more time with patients and focus on preventive, relationship-driven care. Key Responsibilities: Evaluate and treat patients according to established standards of care Coordinate patient services including specialty referrals, hospital coordination, SNF coordination, home health, and durable medical equipment Participate actively in daily care team huddles Maintain accurate and timely medical documentation Collaborate with clinical leadership on quality outcomes and patient care initiatives Deliver patient-centered care in a value-based care environment Manage a senior patient panel of 500 or fewer patients See approximately 8-13 patients per day Education, Licensure, and Experience: Graduate of an accredited MD or DO program Active and unrestricted medical license or willingness to obtain licensure in Maryland Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine Active DEA license Medicare and Medicaid provider numbers preferred Strong interpersonal and communication skills Comfortable working in a team-based care environment Experience or interest in value-based care models preferred Experience with EMR systems preferred Knowledge of HEDIS/STARS, CAHPS, and HOS preferred Experience working with geriatric or underserved populations is a plus Benefits: CME Allowance and CME Days Occurrence-Based Malpractice Insurance 401(k) Retirement Plan Health, Dental, Life, Short-Term and Long-Term Disability Insurance Paid Time Off and Holidays Minimal Call Schedule For more information please reach out to Kuldeep Singh: at View phone number on click.appcast.io or by Email: at View email address on click.appcast.io
