Overview Registered Nurse (RN) - Bronson At Home (Home Health Care) - PRN The Registered Nurse provides skilled nursing services to home care clients according to a physician prescribed plan of care and the Medicare Conditions of Participation. RNs work under the direction of the HH Clinical Services Manager and, under general supervision, manage care delivery to achieve quality outcomes by utilizing the nursing process and coordinating care with other professionals. The RN documents during each visit and communicates with other disciplines as needed related to the patient’s plan of care. Responsibilities Collect comprehensive data pertinent to the patient’s health or situation. Assess the patient’s physical and developmental needs and analyze assessment data to identify appropriate patient needs. Document nursing history and age-appropriate physical/emotional needs/strengths for assigned patients; review vital signs to assess condition and detect deviations from normal. Develop and implement a plan of care using clinical pathways when appropriate; coordinate discharge planning and collaborate with other team members. Administer prescribed medications and treatments in accordance with approved nursing techniques and policies; delegate appropriate tasks to patient care assistants and universal service assistants when applicable. Plan and educate patients and families, and coordinate care with the continuum of care available. Evaluate progress toward expected outcomes and adjust the plan of care as needed; document patient responses to care and outcomes. Deliver care that preserves patient autonomy, dignity, confidentiality, and rights; act as a patient advocate and integrate ethical considerations in practice. Collaborate with multiple health disciplines to effect positive patient outcomes and integrate research findings into practice as appropriate. Participate in quality improvement activities and maintain ongoing professional development and competency records. Qualifications Diploma, Associate’s or Bachelor’s degree in Nursing required 2 years of current continuous experience in an acute care setting preferred Licensed RN in good standing with the State of Michigan required Current driver’s license in the state of Michigan and proof of insurance required BLS certification required Readiness to drive own vehicle on a regularly scheduled basis to perform business for Bronson; pass MVR check after offer and before start date; provide valid auto insurance for the vehicle driven on duty Essential Tasks and Work Environment Ability to read, interpret, and analyze data from various computer systems and equipment to inform plan of action Ability to communicate effectively verbally, in writing, and electronically Physical ability to regularly lift up to 50 pounds and assist with heavier tasks as needed Standards Of Professional Performance Evaluate the quality and effectiveness of nursing practice and participate in quality improvement activities Maintain professional records and participate in ongoing educational activities Demonstrate competency aligned with current nursing practice and professional standards Provide culturally sensitive, age-appropriate care and engage in self-evaluation and peer review Collaborate with patients, families, and health disciplines to achieve positive outcomes Lead in professional practice settings and participate in committees as appropriate Maintain compassionate and caring relationships and contribute to a healthy work environment Incorporate ethical provisions and patient autonomy into all aspects of care Shift First Shift Time Type Part time Scheduled Weekly Hours 10 Cost Center 7310 Home Care Nursing (BAH) Policy Bronson Healthcare Group and its affiliates strictly prohibit the acceptance of unsolicited resumes from recruiters or third-party agencies in response to job postings or word of mouth.
