Physician Assistant (Primary Care)

Major Duties The successful candidate will have a high level of medical knowledge and possesses all the qualities for providing comprehensive and compassionate care for veterans. The position requires excellent interpersonal and leadership skills and offers input into projects pertaining to the development of the clinic. Responsibilities include: Providing comprehensive primary‑care services to a defined panel of patients, including history taking, physical examinations, ordering laboratories and diagnostic studies, diagnosing conditions, and formulating treatment plans for acute and chronic illnesses within the scope of medicine. Conducting clinical examinations, diagnosing and treating acute and chronic diseases, offering preventive care, and coordinating care for the primary‑care panel. Documenting patient care accurately in clinical records. Referring patients to specialists when additional input or treatment beyond the provider’s scope is required. Monitoring PACT teamlet performance to assess adherence to VA standards, assure quality and effectiveness of care, and meet performance measures. Leading the team in assisting patient advocates and administration in resolving patient concerns and inquiries. Exercise medical decision‑making and provide a broad range of diagnostic and therapeutic services for a specific group of patients. Maintaining excellent communication and collaboration with staff and the community to elicit cooperation from others. Serving as a member of a multidisciplinary treatment team that includes social work, pharmacy, mental health, nutrition, and other services. Performing additional duties as assigned. The provider also serves as the leader of a Patient-Aligned Care Team (PACT) composed of RN, LPN, and clerical staff, which delivers direct care for veterans enrolled in primary care.
