PRN Social Worker II: Hospital Discharge & Care Coordination

University Hospitals in Elyria is seeking a PRN Social Worker 2 to join the Elyria Medical Center team. You will collaborate with the health care team to assess psychosocial and discharge planning needs and help formulate patient-centered discharge plans. The role supports patients with mental health issues, provides linkage to community resources, and serves as a liaison to community agencies while guiding staff and interns in complex psychosocial cases. #J-18808-Ljbffr University Hospitals
