Job Description & Requirements Jackson Therapy Partners is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a travel job in Portland, Oregon. Specialty Physical Therapist Discipline Therapy Assignment Details Start Date: 05/18/2026 Duration: 13 weeks Hours: 40 hours per week (8 hours per shift) Shift: 8 hours Employment Type: Travel Facility Type: Skilled Nursing Facility Location: Portland, OR Primary Responsibilities Help patients recover from injuries and manage chronic conditions in a skilled nursing facility setting. Provide fall risk management, transfers, and mobility training. Document all patient care in electronic medical records.
