Registered Nurse - Hospice

Job Description

Job Summary:

Provides specialized hospice nursing care to patients in their homes, coordinating services according to physician orders and agency protocols. Responsible for overseeing patient care plans, documenting clinical activities, and supervising home health aides and LPNs/LVNs to ensure quality care delivery.

Responsibilities:

• Coordinate and deliver hospice nursing care to assigned patients in their homes.
• Develop, implement, and evaluate individualized care plans based on patient needs and physician instructions.
• Supervise and evaluate the performance of Home Health Aides and Licensed Practical/Vocational Nurses.
• Document patient assessments, care provided, and clinical progress accurately within established timeframes.
• Travel up to 75 miles from the home clinic to deliver patient care and attend periodic meetings.
• Participate in on-call rotations, providing backup support for patient emergencies and urgent care needs.
  Qualifications:
• Current RN license in the state of practice with no restrictions.
• Minimum of 1 year of recent hospice or adult home health experience.
• Proficient in electronic medical record systems, preferably HomeCare HomeBase (HCHB), and familiar with OASIS documentation.
• Valid driver's license, reliable transportation, and agency-required liability insurance.
• Ability to travel within assigned territory and manage a flexible schedule.
• Strong clinical skills in hospice care, wound management, and patient/family support.
