Family Practice/Primary Care Nurse Practitioner

House call service provider in NJ is looking for NPs for 3 months contract. Please see the details below and let us know if you'd be interested.

Start Date: June 10th or 24th

Duration: 90-day assignment

Location: Home visits across Ocean County (6 providers needed), Middlesex County, and Morris County, NJ please specify county preference upon submission

Scheduling:
40 hours/week, Monday through Saturday (flexible)
8-hour shift scheduled between 8 AM 8 PM (e.g., 8 5, 12 8)
Includes a 1-hour lunch break
Schedules released 2 3 weeks in advance

Job Details:
Conduct 5 6 home health assessments daily
Paid for full day regardless of patient volume
Responsibilities: annual risk assessments, chronic condition reviews
No prescribing or treatment, assessment-only role
Escalate urgent issues to PCP
If applicable, perform A1C tests (no other screenings)
Travel required mileage reimbursed daily

Equipment Provided:
iPad
Blood pressure cuff
Lab kits

Credentialing:
Fast-track credentialing (1 2 weeks)
Must hold an active NJ license and NJ Medicare number

Requirements :
Board-Certified FNP or ACNP
Licensed in NJ
Must have NJ Medicare number
Travel NOT covered

Malpractice insurance will be covered.
