Foster Care Case Manager

OrthoNY, LLP

OrthoNY is seeking a dedicated Case Manager in Syracuse, NY to support orthopedic patients through the continuum of care. You will work within the ambulatory surgery program, coordinating pre- and post-operative activities and serving as the main contact for patients and providers. The role requires 2–3 years nursing or telephonic case management experience, with a preference for orthopedic exposure. Strong communication and EMR proficiency are essential, and travel to SOS locations may be #J-18808-Ljbffr
