Responsibilities:

• Provide personal care and support services to patients, including assistance with daily living activities, such as bathing, dressing, and grooming
• Assist with mobility and transfers, including use of wheelchairs, walkers, and canes
• Administer basic care, such as taking vital signs, feeding, and toileting
• Perform light housekeeping and laundry tasks to maintain a clean and safe environment
• Assist with medication reminders and administration (as permitted by state law)
• Collaborate with a multidisciplinary healthcare team to develop and implement individualized care plans
• Educate patients and families on self-care techniques and disease management
