Tryfacta is seeking a Long Term Care (LTC) for our client in Thorp, WI 54771. This is a 13 weeks contract assignment. If you meet the qualifications listed below and are interested, please apply now!

Location: Thorp, WI 54771

Duration: 13 Weeks

Work Schedule: Variable Day and Evening Shifts (8-Hour and 16-Hour Shifts)

Key Responsibilities:

• Provide direct nursing care to residents in a Skilled Nursing Facility setting.
• Administer medications and treatments as prescribed.
• Monitor resident conditions and report changes to the healthcare team.
• Maintain accurate patient records and documentation.
• Assist with resident assessments and care planning.
• Collaborate with physicians, RNs, CNAs, and interdisciplinary care teams to ensure quality patient care.

Requirements:

• Active LPN License required.
• Long Term Care (LTC) and/or Skilled Nursing Facility (SNF) experience preferred.
• Strong clinical assessment and documentation skills.
• Excellent communication and teamwork abilities.
• Ability to work flexible day and evening schedules.
