Tryfacta is seeking a Long Term Care (LTC) for our client in Thorp, WI 54771. This is a 13 weeks contract assignment. If you meet the qualifications listed below and are interested, please apply now!
Location: Thorp, WI 54771
Duration: 13 weeks
Work Schedule: Variable day and evening shifts (8-hour and 16-hour shifts)
Key responsibilities:
- Provide direct nursing care to residents in a skilled nursing facility setting.
- Administer medications and treatments as prescribed.
- Monitor resident conditions and report changes to the healthcare team.
- Maintain accurate patient records and documentation.
- Assist with resident assessments and care planning.
- Collaborate with physicians, RNs, CNAs, and interdisciplinary care teams to ensure quality patient care.
Requirements:
- Active LPN license required.
- Long term care (LTC) and/or skilled nursing facility (SNF) experience preferred.
- Strong clinical assessment and documentation skills.
- Excellent communication and teamwork abilities.
- Ability to work flexible day and evening schedules.
