Contract Type: Travel Contract
Shift Type: Monday - Friday, 8:00 AM - 4:00 PM (Day shift, 40 hours/week)
Hours per Week: 40
Candidate Pay Rate: $49 per hour

Roles & Responsibilities
- Screen, evaluate, and treat patients to restore function and mobility
- Develop and update individualized treatment and discharge plans of care
- Document services accurately and timely to support continuity of care
- Supervise Physical Therapist Assistants and Rehab Techs per state practice acts
- Collaborate with interdisciplinary care teams and families to optimize patient outcomes
- Maintain compliance with safety, infection control, and regulatory standards
Qualifications
- Graduate of an APTA-accredited Physical Therapy program (DPT, MSPT, or BSPT)
- Active Indiana state PT license (or ability to obtain prior to start)
- Strong communication, documentation, and clinical skills
- Flexibility to adapt to varying facilities, schedules, and patient needs
Education
- Graduate of an APTA-accredited Physical Therapy program (DPT, MSPT, or BSPT)
Knowledge & Skills
- Physical therapy evaluation and treatment planning
- Patient evaluation and functional mobility assessment
- Accurate and timely clinical documentation
- Interdisciplinary communication and collaboration
- Knowledge of state and federal regulations and facility guidelines
- Adaptability to different rehab settings and patient populations
