Equity Staffing is seeking an experienced Hospice Registered Nurse Case Manager (RNCM) to join a compassionate care team supporting patients and families throughout the hospice journey. This role focuses on providing skilled nursing care, symptom management, patient education, and emotional support in a home hospice setting throughout the Flagstaff, Arizona coverage area. Position Details Location: Northern Arizona Hospice - Flagstaff, AZ Schedule: Monday-Friday, 8:00 AM-4:30 PM Weekly Guarantee: 40 hours Coverage Area: 65-mile radius On-call/Weekend Requirement: As needed for backup on‑call support Key Responsibilities Provide hospice nursing care and case management for assigned patients Perform symptom management and medication management Complete hospice admissions, recertifications, PRN visits, and symptom follow-ups Educate patients and families on disease progression and the dying process Deliver compassionate personal care support and emotional guidance Coordinate care plans with interdisciplinary hospice team members Maintain accurate and timely clinical documentation Required Qualifications Active Registered Nurse (RN) license Previous hospice nursing experience required Strong clinical assessment and communication skills Ability to work independently in a home health/hospice environment Preferred Qualifications Experience with Homecare Homebase (HCHB/EMAR) Compassionate bedside manner and strong family support skills If you are a compassionate RN with hospice experience looking to make a meaningful impact in patients' lives, we encourage you to apply today. #J-18808-Ljbffr
