Case Manager - RN

Shift Monday - Friday (8:00am - 4:30pm) Location Singing River Gulfport Hospital Position Overview The Case Manager focuses on clinical quality and resource management resulting in defined clinical satisfaction and financial outcomes. They coordinate patient care to a population of different ages within the Singing River Health System and ensure all duties are in accordance with the organization’s procedures, policies, accreditation organization, and governing guidance. Case Managers must work effectively under pressure, multitask, and be self‑directed, with strong written and oral communication skills, analytical and critical thinking skills, conflict management, team‑building, and organizational skills. They participate in daily discharge‑planning meetings to address barriers to discharge, target successful transitions from one level of care to the next, and create seamless communication between inpatient and outpatient case‑management teams. Management reserves the right to revise the job or require other or different tasks as assigned. Education Graduate of an accredited School of Nursing; Bachelor’s Degree in Nursing preferred. License Currently licensed to practice as a Registered Nurse in the state of Mississippi. Certification Must complete and maintain BCLS certification upon hire and/or transfer. TB certification preferred. Must have de‑escalation training completed by the end of position orientation (90 days) and maintain appropriate level of de‑escalation training. Experience Two (2) years’ nursing experience in ambulatory or acute care relevant to the practice of Case Management. Reports To Director‑ Patient Resource Management Supervises None. Physical Demands Moderately active: sitting with frequent movement around the office and facility, traveling within the SRHS service area, and exerting negligible force to lift, carry, push, pull, or move objects, including human bodies. Exposure to body fluids, sputum, and tissues that may carry infectious disease; repetitive motions of wrists, hands, or fingers while using standard office equipment; and visual acuity required to shift focus between computer monitors and printed materials. Must be active for extended periods and flexible to schedule changes. Mental Demands Demonstrate keen assessment and decision‑making abilities, superior communication and enunciation skills for in‑person and telephone interactions, strong written and verbal communication, emotional stability for high‑stress levels, and ability to work under pressure to meet deadlines. Requires detail orientation, multitasking in complex situations, and collaborative working relationships throughout SRHS and other organizations. Special Demands Superior customer service skills and professional etiquette; proficient use of computer and office technology, including MS Outlook and Word. Demonstrate appropriate clinical judgment and professional skills for a patient population of all ages.
