Physical Therapist (PT) — Holyoke, Massachusetts

Location: Holyoke, Massachusetts
Compensation: Up to $63 per hour based on unit rate
Schedule: 30 hours per week
Hours: Monday–Friday
Weekend Rotation: None
Setting: Mobile Outpatient Therapy

Overview

We are looking for a motivated Physical Therapist (PT) to join a mobile outpatient therapy team serving patients in and around Holyoke, Massachusetts.

This role is designed for a clinician who enjoys delivering personalized care while having the flexibility and variety that comes with a mobile therapy model. Rather than treating every patient in the same traditional clinic environment, you'll meet patients where they are and tailor treatment to their individual needs and goals.

The position offers 30 hours per week, Monday–Friday, with no weekend rotation , creating a schedule that provides both professional fulfillment and consistency.

Responsibilities

• Conduct comprehensive patient evaluations

• Develop individualized plans of care

• Provide skilled physical therapy interventions based on patient needs

• Help patients improve strength, balance, mobility, endurance, and functional independence

• Track patient progress and modify treatment plans appropriately

• Provide patient and caregiver education

• Develop appropriate home exercise programs

• Maintain timely and accurate documentation

• Coordinate with other healthcare professionals when appropriate

• Travel between patient locations as required by the mobile therapy model

Qualifications

• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program

• Current Massachusetts Physical Therapist license

• Strong communication and interpersonal skills

• Ability to work independently while maintaining high clinical standards

• Organized and adaptable approach to patient care

• Comfortable providing therapy in a mobile outpatient environment
