This environment promotes a balance of independence and collaborative care, ideal for achieving optimal patient outcomes.Key Responsibilities\n\n Participate in mental health and medical rounds to evaluate client needs and treatment plans collaboratively.\n Engage in case conferences to tailor individualized treatment strategies for your clients and discuss progress.\n Conduct thorough mental health evaluations, crisis interventions, and follow-up care to ensure continuity of care.\n Perform psychological testing, if assigned, to guide clinical decisions and refine treatment approaches.\n Coordinate discharge planning services with Reentry Social Workers, ensuring a seamless transition for patients returning to their communities.\n Document all clinical activities accurately and timely, complying with state, federal, and organizational guidelines.\n Lead and facilitate group therapy sessions, emphasizing skill-building and emotional support in a structured environment.\n Work closely with the Department of Correction to ensure comprehensive care coordination for patients needing supportive housing and treatment.\n Engage in weekly clinical supervision to enhance your skills and pursue ongoing professional development.\nPatient Population / Client Focus\nThis role primarily focuses on children and teenagers, providing therapeutic support tailored to their unique mental health needs. Clients may face a range of challenges, including anxiety, depression, trauma, and family dynamics.
