Grand County RN - Part Time (Weekend/Weekday Flexibility)

Overview Namaste Home Health & Hospice is looking for a flexible and experienced Registered Nurse to join our team in Grand County, CO. The role will start at two days per week: one weekday and one weekend day, with potential for growth. Responsibilities Completes initial, comprehensive and ongoing assessments of patients and families to determine hospice needs. Provides a complete physical assessment and history of current and previous illnesses. Implements professional nursing care using all elements of the nursing process. Writes and initiates the plan of care, regularly re-evaluates patient and family needs, and revises plans as necessary. Uses health assessment data to determine nursing diagnoses and develops care plans that incorporate nursing actions. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Counsels patients and families on nursing and related needs, providing health care instructions per assessment and plan. Assists patients with activities of daily living, facilitates self-sufficiency and optional comfort care. Acts as case manager, coordinating care for an assigned caseload. Maintains accurate clinical notes, records pain/symptom management changes, and reports patient condition changes to physicians. Communicates with physicians, community health personnel, and interdisciplinary group members to coordinate the care plan and provide education to patients and families. Provides a safe and effective environment for patients and assists in continuity of care. Attends interdisciplinary group meetings and supports the emotional needs of patients and families. Additional Duties Participates in on-call duties as defined by policy. Ensures arrangements for equipment and other necessary items and services are available. Supervises ancillary personnel and delegates responsibilities when required. Participates in quality assessment, performance improvement teams, and education activities. Requirements Graduate of a National League for Nursing accredited school of nursing. Minimum two years of experience, with at least one year in public health, home care, or hospice nursing preferred. Registered nurse licensure current in the state of Colorado. Demonstrated excellent observation, problem solving, verbal and written communication skills. Ability to organize and prioritize workload independently. Physical ability to perform prolonged walking or standing, lift, position and transfer patients, and other physical requirements. Current CPR certification. Licensed driver with an insured automobile in good working order. Pay range: $45–$50 per hour.
