Responsibilities Provide direct patient care to residents, including administering medications, wound care, and IV therapy as needed. Develop and implement individualized care plans in collaboration with the interdisciplinary team. Conduct regular assessments of patients' physical and emotional needs, and document all care provided. Collaborate with other healthcare professionals to ensure seamless communication and coordination of care. Participate in maintaining a safe and clean environment for residents and staff. Demonstrate a commitment to continuous learning and professional development. Requirements Current licensure as a Registered Nurse in the State of New York. Prior experience in a long-term care setting preferred.
