DIRECT SUPPORT PROFESSIONAL

JOB SUMMARY

The Direct Support Professional will be responsible for the day to day care for the individual(s) assigned to his/her oversight.

NON-EXEMPT position

1. Acts a positive role model and communicates effectively. 
2. Completes tasks accurately on time.
3. Accommodates shift changes to meet individual served/staff ratio as required.
4. Assists the person served in making community connections to ensure a presence in their community. 
5. Manages a crisis or responds to emergency medical situations.
6. When appropriate, assists individuals served with activities of daily living (self-administration of medication, hygiene and personal care, nutritional oversight, home duties, supervision on medical appointments and community networking, etc.).
7. Implements Behavioral Management and Safety Plans as well as documents data.
8. Effectively carries out ISP goals. Meets frequently with case manager and documents progress notes and goal data.
9. Performs household cleaning and general management duties and environmental checks.
10. Operates a motorized vehicle and provides transportation for the person served to public settings, provides support and supervises for safety. 
11. If driving an agency-owned or state-owned vehicle, responsible for attending and passing training to adequately inspect vehicles and learn the correct way to report problems. Also responsible for inspecting vehicles prior to each trip and completing the Daily Vehicle Inspection Report as described in the policy. If driving a vehicle equipped with a lift, responsible for completing the daily Vehicle Inspection for Vehicles Equipped for ADA Report. Problems/ and or damages should be documented on the Vehicles Problems Report and submitted to the Transportation Director immediately. 
12. Maintains confidentiality
13. Maintains all mandatory required training to adhere to compliance regulations.
14. Completes reports as required. 
15. Accurately and completely document all services on the day they occur. 
16. Submit all documentation to immediate supervisor on a weekly basis or as needed.
17. Attend all required staff meetings as scheduled. (ISP, addendum meetings, house meetings, etc.) 
18. Provides positive support and assistance to individual(s) served in achieving goals outlined in their ISP.
19. Educates and empowers the individual(s) served to become the person they desire to be.
20. Effectively communicates with the assigned case manager of the individual(s) served to ensure continuity of care. 
21. Tracks, records, and documents all aspects per the individual(s) served ISP/BSP/SP. 
22. Reconciles and documents the individual(s) served spending/grocery money at the beginning and end of the shift. 
23. Ensures the individual(s) served attends the HRST Clinic on a monthly basis will all pertinent documentation.
24. Records and documents on all required documentation, including but not limited to the Shift Communication log, shift change form, temperature log, visitors log, etc.
25. Maintains data recording individual’s progress and reports weekly to Residential Case Manager. Maintains documentation in Therapy, QuickMar, any healthcare tracking on each shift.
26. Transports the individual to medical/dental appointments, completes the correct documentation and scans the documentation to the appropriate place and people.

REQUIRED COMPETENCIES:

1. Ability to read, write and comprehend English, communicate clearly and effectively.
2. Ability to walk, bend, stoop, climb, stand, and lift, up to 50 lbs. using proper techniques (if lifting an individual is required)
3. Ability to understand and follow written and oral direction. 
4. Ability to pass a criminal background check, drug screening, and MVR check.
5. Ability to show competency in all trainings.
6. Other duties as assigned.

QUALIFICATIONS (required):

1. High School diploma
2. a minimum of 1 year experience working with individuals with Developmental Disabilities, preferably in a behavior-focused residential home. 
3. Must be at least 21 years old
4. Must possess a valid Georgia’s Driver’s license
5. Reliable transportation
6. CLEAN MVR
7. Must possess a valid phone number that remains active
8. Must be familiar with operating a calculator, computer, fax, Hoyer lift equipment, ALL household appliances
9. Must possess excellent communication skills

OTHER KNOWLEDGE AND ABILITIES:

He/she must be friendly, caring, and have good communication and organizational skills in order to prioritize duties and meet deadlines as they arise. He/she must have a personal vehicle and a valid GEORGIA’S driver’s license. He/she must have working knowledge of the Albany Advocacy Resource Center’s Mission and Vision as well as the agency’s programs and services. He/she must have knowledge and understanding of the agency’s accreditation process and its value to the organization.

TYPICAL WORKING CONDITIONS:

Daily working conditions will change depending on the schedule for any given day and DSP staff are required to accept rotating shifts and schedules based on the served/staff ratio.

JOB RELATIONSHIPS:

He/She will be supervised by the case manager.
