It involves analyzing patient responses, monitoring progress, and adjusting care plans accordingly. Responsibilities include developing and implementing individualized treatment plans, conducting assessments using various tools, and documenting interventions for continuity of care. The role requires collaboration with patients, families, and healthcare team members, as well as educating patients and caregivers about treatment and resources. It also involves organizing caseloads, maintaining clinical records, participating in multidisciplinary team meetings, and engaging in community outreach and professional development activities. Qualifications include a Bachelor’s degree in Physical Therapy, 1-2 years of clinical experience, a valid PT license, a driver’s license, CPR certification, and strong clinical, organizational, and communication skills.
