Responsibilities

• Provide physical therapy services under the supervision of the licensed PT, implementing and adapting treatment plans for LTAC patients.
• Assist with patient evaluations, therapy sessions, and progression of therapeutic activities, including gait training, balance, strength, and range-of-motion exercises.
• Monitor patient responses to therapy, document progress, and communicate changes to the supervising PT and interdisciplinary team.
• Support safe patient handling, transfers, and mobility aids; ensure appropriate use of equipment and adherence to infection control protocols.
• Collaborate with nursing, rehabilitation, and discharge planning teams to coordinate care and optimize patient outcomes.

Requirements

• Active Physical Therapy Assistant license in New Hampshire or eligibility for licensure
• Graduation from an accredited PTA program
• Experience in a LTAC, SNF, or inpatient rehabilitation setting preferred
• Ability to work the listed shift (08:00–16:30) and travel to Lebanon, NH
• Strong communication skills and ability to collaborate within an interdisciplinary team
