- Conduct assessments for patients in both inpatient and outpatient settings, creating personalized treatment strategies
- Implement physical therapy techniques focused on enhancing mobility, strength, balance, and overall function
- Work collaboratively with nurses, physicians, case managers, and other rehabilitation experts to maximize patient outcomes
- Provide education to patients and their families regarding therapeutic objectives, discharge processes, and home exercise regimens
- Ensure thorough documentation of evaluations, treatments, and patient progress according to facility and regulatory standards
- Engage in team care rounds and interdisciplinary meetings to maintain coordinated patient care
- Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution
- State licensure as a Physical Therapist
- CPR certification preferred
- Specialized training (such as geriatric or neurological certifications) is a bonus, but not mandatory
- Preferably previous experience in a similar role
- Capability to manage patients with multifaceted medical and mobility challenges
- Excellent communication and teamwork skills
- Proficiency with digital charting and clinical documentation systems
