LVN/LPT Position at Evolve Treatment CentersEvolve Treatment Centers is hiring for a full-time LVN/LPT to oversee our two facilities in Agoura Hills and Calabasas CA! Candidates must be eager to participate in an environment that promotes continuous learning and works as a member of a team.We are looking for candidates that can stay calm during stressful situations, those that take initiative and maintains appropriate boundaries with clients.Essential Functions:Direct patient care and assessment; including the reporting of observed changes in mental status and/or physical condition, response to emergencies, and safety risksCompletes documentation in an efficient and timely manner, including nursing assessments, transcription of M.D.s' phone ordersKnowledge of medication administration records, vital sign monitoringAttendance and participation in terms of treatment planning and professional staff meetingsCall in medications to pharmacy and pick upProvides in service training to technical staff on medications, documentation, and monitoring of vital signsProvides ongoing patient education on all aspects of medications and other health issues including hygieneMonitors supplies and reports needs to Facility ManagerEstablishes a compassionate environment by providing emotional, psychological, and educational support to patients and familiesResolves patient problems and needs by utilizing multidisciplinary team strategiesMaintains safe and clean working environment by complying with procedures, rules, and regulations; calling for assistance from health care support personnelProtects patients and employees by adhering to infection-control policies and protocols, medication administration and storage proceduresMaintains continuity among nurses by documenting and communicating actions, irregularities, and continuing needs to weekend nurseKnowledge of:Psychiatric medications and their side effectsBasic first aid and life-saving skillsAbility to:Speak with clients and parents in a compassionate and patient mannerMaintain professional boundaries with clients and staffAssess emergency situations and delegate responsibilities to staffWork with a multi-disciplinary teamExercise independent judgment and leadership, analyze situations accurately and take effective actionMake decisions and handle difficult situations with tact and professionalismRequirements:Valid California Nursing License LVN/LPTCurrent CPR/ BLS cardMinimum of one year experience as a working nurse. Psych or chemical dependence background preferredGood communication skills and basic computer literacyThis is a full-time, onsite position, Mon - Fri 8:30 am - 5pm.
