Position Summary

The Licensed Vocational Nurse (LVN) will support outpatient clinical operations with a focus on urinalysis testing, contingency management, and patient care coordination. This role blends clinical nursing duties with administrative and patient-facing responsibilities, ensuring accurate documentation, timely communication of results, and high-quality patient engagement.

Key Responsibilities

Urinalysis & Contingency Management

• Conduct observed and non-observed urinalysis collections in accordance with program protocols
• Perform point-of-care testing and ensure accuracy of results
• Log, track, and maintain detailed records of all toxicology screenings
• Communicate test results to patients in a professional, supportive manner
• Implement contingency management protocols, including tracking compliance
• Ensure proper chain-of-custody procedures and compliance with regulatory standards

Patient Care & Clinical Duties

• Obtain and record vital signs (blood pressure, pulse, temperature, etc.)
• Administer medications as ordered, including injections (if applicable)
• Assist with medication management and reconciliation under provider supervision
• Support medical providers during patient visits and procedures
• Provide patient education related to treatment, medications, and program expectations

Admissions & Assessments

• Assist with facility intakes, including gathering medical history and documentation
• Support completion of ASAM assessments and ensure proper charting

Administrative & Clerical Support

• Maintain accurate and timely documentation in the electronic medical record (EMR)
• Coordinate with clinical and administrative teams to ensure continuity of care
• Manage scheduling, patient follow-ups, and general office tasks
• Handle phone calls, patient inquiries, and front-office support as needed

Qualifications

• Current, active LVN license in the state of California
• Experience in outpatient, behavioral health, or substance use treatment preferred
• Knowledge of urinalysis/toxicology procedures and chain-of-custody protocols preferred
• Familiarity with ASAM criteria and intake processes is a plus
• Strong documentation and organizational skills
• Ability to communicate effectively with patients and multidisciplinary teams
• Proficient in EMR systems and basic computer applications
