As a member of our rehabilitation team, you will evaluate patients within both inpatient and outpatient settings, crafting individualized treatment plans tailored to their unique needs. Your expertise will guide them through tailored physical therapy interventions aimed at improving their mobility, strength, balance, and overall function. You won't be alone in this endeavorcollaborating closely with nurses, doctors, case managers, and other rehabilitation professionals, you will work together to optimize patient outcomes. Communication will be essential in your role as you educate both patients and their families about therapeutic objectives, discharge planning, and exercises to continue at home. Your meticulous documentation of evaluations, treatments, and progress will align with both facility and regulatory guidelines. Active participation in patient care rounds and interdisciplinary conferences will ensure that you are part of a seamless continuum of care.

To succeed, you'll need to possess a Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program, along with state licensure as a Physical Therapist.
