Resident Care Coordinator

Resident Care Coordinator – Cameron Hall – Canton The Resident Care Coordinator is responsible for the coordination of resident care needs, including drafting schedules, maintaining required logs (physician appointment and pharmacy logs), filing and organizing clinical records, retrieving signed physician orders, and assisting the Director of Health & Wellness with training new Care Managers and Medication Technicians. In the absence of the Director, the Coordinator assumes supervision responsibilities and ensures compliance with TerraBella expectations, policies, procedures, and State regulations. Responsibilities Ensure resident care staffing meets regulatory and company requirements; submit a 4‑week rolling schedule to the Director of Health & Wellness at least one week before posting. Maintain the resident care scheduling book with approved time‑off requests noted. File all documents relevant to the resident record in chronological order and perform a quality‑assurance review to verify dates, signatures, and physician verification. Retrieve verbal order signatures, ensuring all orders are returned signed by the physician within 14 days of receipt. Conduct weekly cart audits of medication carts when the Support Nurse is unavailable; submit audit sheets to the Director of Health & Wellness and Executive Director. Assist with data collection and reporting processes as requested. Review medication administration process daily to ensure medications are passed timely and accurately; intervene as needed to provide support or training during the med pass. Pre‑screen new team members, ensuring they are fully compliant with all applicable policies before completing orientation. Process physician’s orders following the medication order procedure and maintain records of order transcription and verification. Protect residents’ rights. Provide emergency treatment when required and notify physician and the family or responsible party; complete incident and accident reports. Notify the Director of Health & Wellness of changes in a resident’s condition or unusual observations; if the Director is absent, notify the physician directly. Document changes in the resident’s record and the 24‑hour shift report. Schedule appointments and arrange transportation for diagnostic, therapeutic, dental, or medical services; ensure compliance with the physician visit log process. Understand and implement the Fire Safety and Disaster Plan and evacuation procedures during drills and emergencies. Perform all other duties assigned by the Director of Health & Wellness. Review the monthly Medication Administration Record verification to ensure orders are compliant. Assist in ensuring compliance with all required logs, policies, and procedures relevant to medication administration. Ensure care staff understand their role regarding dietary services, including serving, water pouring at every meal, and correct dietary preparations. Assist in serving residents during meal time and circulate through AL and Memory Care dining rooms. Conduct building rounds at the beginning and end of each shift to evaluate care and meet resident needs. Be available to work overtime in emergency situations. Set a positive example for residents and staff in appearance, conduct, and interest. Participate in weekend call rotation with other managers. Participate in the Manager on Duty Program. Qualifications Meet minimum criteria to perform as a Supervisor in Charge in the State of North Carolina. Licensed Medication Technician, CNA with at least one year of experience preferred. Excellent written and verbal communication skills. Demonstrated interest in working with a senior population. Warm, outgoing, and compassionate personality. Deep moral integrity with demonstrated maturity and leadership skills. Knowledge of community agencies and appreciation for the social and life history status of residents.
