Brief Job Description This position is available for Morning, Evening and Night shifts on multiple homes and may earn additional pay for evenings, nights, weekends, or maximum security locations. The Direct Care RN performs routine nursing work at a State Supported Living Center (SSLC) setting, evaluating, treating, and documenting healthcare needs. The role collaborates with direct care staff, physicians, and other nurses to ensure high quality care. The RN also assists LVNs, certified medication aides, and participates in quality assurance initiatives. Essential Job Functions (EJFs) Attend work on a regular basis and may be required to work rotating or extended shifts in accordance with agency policy. Provide routine nursing tasks and procedures (tracheostomy care, gastrostomy/jejunostomy care, wound care, breathing treatments, specimen collection, screening tests, medication administration, immunizations, and acute care plans). Monitor individuals’ weight, diet, feeding, vital signs, and signs of illness, using nursing judgment to initiate appropriate interventions. Document and report treatments and observations accurately in the electronic health record or paper chart. Assist with services provided by LVNs and CMAs, offering recommendations for improvements and ensuring staff competency. Support the Nurse Manager with controlled drug counts, medication counts, emergency checklists, temperature logs, and glucose controls. Implement infection control measures, participate in quality assurance programs, and respond to emergency situations. Perform additional duties such as disaster response, continuity of operations support, or other assigned tasks requiring alternate shifts or locations. Knowledge, Skills and Abilities (KSAs) Knowledge of nursing techniques, health‑care laws, medical diagnoses, and treatment programs. Knowledge of state and federal regulations, ICF‑IID rules, and the Settlement Agreement. Skill in caring for patients, including use of computer/software and electronic health records. Excellent verbal and written communication and the ability to present ideas clearly. Ability to work independently and productively with all levels of staff. Ability to provide support and coaching to LVNs, CMAs, and DSPs. Ability to make decisions and recommendations to SSLC leadership, including on‑call administrators. Registrations, Licensure Requirements or Certifications Licensed to practice as a registered nurse in the State of Texas or a state that participates in the Nurse Licensure Compact. Initial Screening Criteria Work experience as a practicing registered nurse is required. At least six (6) months of experience in a health agency, hospital, or health‑care facility is preferred. Bachelor's Degree is preferred. Additional Information Applicants must pass a fingerprint criminal background check, pre-employment drug screen, and registry checks (CANRS, Nurse Aide, Medication Aide, and LEIE). All State Supported Living Center employees are subject to random drug testing. Flexibility in work hours, overtime, weekends, and holidays may be required.
