Job Title: Wound Care RN Facility: Spring Creek Rehabilitation & Nursing Care Center Location: Brooklyn, NY Shift: Full-Time, 9:00 AM – 5:00 PM Salary: $115,000 per year Job Description Spring Creek Rehabilitation & Nursing Care Center is seeking an experienced and dedicated Wound Care RN to join our nursing team. In this full-time role, you will provide expert wound care to our residents while collaborating with a multidisciplinary team to ensure optimal healing, comfort, and quality of life. Key Responsibilities Assess residents with existing or newly developed wounds. Collaborate with physicians to establish and implement individualized treatment plans. Lead weekly wound rounds with the care team; monitor and document wound progress. Maintain accurate weekly wound tracking forms. Communicate promptly with physicians when wounds are not responding to treatment. Prepare and distribute weekly decubitus statistics to the wound care team. Conduct in-service training for staff on wound care procedures. Perform quarterly Quality Assurance reports to ensure compliance with wound care standards. Observe and evaluate dressing changes to ensure staff competency. Provide education on wound care to residents and families as needed. Participate in continuing education to stay current with best practices.
