What You'll Do

• Evaluate residents' functional abilities and therapy needs
• Create and implement individualized treatment plans
• Deliver skilled physical therapy services per physician orders
• Document treatments and resident progress accurately and timely
• Collaborate with our interdisciplinary care team
• Educate residents, families, and caregivers
• Ensure compliance with all regulatory and facility standards

What We're Looking For

• Active Kansas Physical Therapist license in good standing
• SNF or long-term care experience preferred
• Strong communication and interpersonal skills
• Familiarity with documentation systems a plus
• Commitment to compassionate, high-quality care
