Physical Therapist
Provide specialized physical therapy services to patients requiring rehabilitation and recovery support. Collaborate with healthcare teams to develop and implement individualized treatment plans that promote optimal functional outcomes.
Responsibilities
- Assess patients' physical condition and establish appropriate therapy goals.
- Design and execute personalized treatment plans based on patient needs.
- Monitor patient progress and adjust therapies accordingly.
- Document treatment sessions and maintain accurate patient records.
- Educate patients and families on exercises and care techniques.
- Collaborate with healthcare staff to ensure comprehensive care delivery.
Qualifications
- Hold a valid state licensure as a licensed Physical Therapist.
- Possess proven experience in a skilled nursing or rehabilitation setting.
- Demonstrate strong assessment and communication skills.
- Ability to work independently and as part of a multidisciplinary team.
- Maintain current CPR and basic life support certifications.
