JOB DESCRIPTION

Job Summary

Provides Occupational Therapy services for residents/clients across Bethesda’s care settings, including the Skilled Nursing Facility, Home Health, and Outpatient programs, including evaluation, treatment, development, and execution of therapy plans, and completion of all required documentation and communication. Works under physician orders and follows established standards of occupational therapy, along with all facility policies, procedures, and state regulations.

Job Relationships

• Reports to: Director of Rehabilitation

• Internal: SNF Administrator, Home Health and Hospice Director, Director of Nursing, Social Services, Human Resources, Accounts Payable, Billing.

• External: Physicians, Medical Director, MDH personnel, CMS surveyors, hospital discharge planners, referral sources, patients and families, and the general community.

Hours of Work: Full-time

Dress Code: Clean, fitted scrubs in solid colors or simple pattern

Essential Requirements of Work: (Minimum qualifications necessary to function at full productivity.)

1. Education:

a) Bachelor of Science in Occupational Therapy

b) Current accredited MN Occupational Therapist License

2. Experience:

a) One to three years of prior experience in a rehabilitation setting, such as acute care, inpatient rehab, outpatient clinics, SNFS, long-term care, assisted living, memory care, or home health.

b) Knowledge of State and Federal regulations.

3. Personal:

a) Highly developed communication skills, both oral and written..

b) Self-motivated with strong attention to detail.

c) Strong problem-solving and decision-making skills.

d) Ability to adapt to various work settings and changing priorities.

e) Ability to work varying hours, including weekends and holidays.

4. Job Responsibilities:

a) Assess & Evaluate Patients: Conduct evaluations to understand residents’ physical, cognitive, and emotional needs affecting daily functioning and quality of life.

b) Develop & Implement Treatment Plans: Create individualized intervention programs with identified goals and methods; coordinate plans with healthcare team, patients, and families; initiate referrals to COTAs as appropriate.

c) Provide Therapeutic Interventions & Adaptive Solutions: Deliver therapeutic activities, exercises, and adaptive techniques; provide/recommend/fabricate assistive devices, orthotics, and prosthetics; train patients, families, and staff in proper use.

d) Educate & Support Patients & Families: Deliver patient and caregiver education on coping strategies, home modifications, discharge planning, and community programming to enhance independence and quality of life.

e) Monitor Progress & Adapt Treatment: Document patient assessments, treatment outcomes, and progress; modify interventions as indicated to achieve established goals per physician orders.

f) Collaborate: Partner with physicians, physical therapists, nursing staff, and other professionals to coordinate comprehensive rehabilitation programming and discharge planning.

g) Maintain Professional Standards: Maintain continuing education and professional development; adhere to infection control, safety standards, and universal precautions; participate in in-service training and team meetings.

Physical Demands of the Job:

1. Standing and walking required for extended periods.

2. Occasional kneeling, stooping, squatting, twisting, and bending.

3. Sitting required for documentation, administrative, and computer-based tasks.

Professional and Personal Growth:

1. Report for and leave the work area on a timely basis.

2. Participate in the annual performance evaluation process.

3. Participate in all required in-service programs at Bethesda.

4. Maintain professional conduct and a positive, collaborative attitude toward staff, patients, and visitors.

5. Be willing to learn and adapt to meet the evolving needs of the organization.
