Location: Upper Northeast Philadelphia & Lower Bucks County Areas, PA (Field-Based / Local Travel)

Job Type: Full-time (Minimum 30 visits per week)

Salary Range: $100,000 - $110,000 / year (Dependent on experience)

Job Description

A growing, clinician-owned home health agency is seeking a dedicated, full-time Physical Therapist (PT) to join our supportive team. This position offers a high level of autonomy with a flexible schedule that allows you to manage your own hours while providing high-quality care to patients within Upper Northeast Philadelphia and Lower Bucks County.

You will be backed by a strong office support team and equipped with modern, user-friendly documentation and communication technology to streamline your field operations. This role offers a steady base caseload with the regular opportunity to take on additional visits for increased earnings.

Key Responsibilities

• Patient Assessments: Perform comprehensive initial and ongoing physical therapy assessments for patients within their home environments.
• Care Planning: Develop and implement individualized, evidence-based treatment plans tailored to patient needs and compliant with physician orders.
• Therapeutic Interventions: Deliver hands-on therapeutic interventions to improve patient mobility, strength, balance, and overall physical function.
• Education & Support: Educate patients, families, and caregivers on home exercise programs, safety modifications, and long-term disease management.
• Clinical Documentation: Accurately and efficiently document patient progress, care plans, and clinical outcomes in accordance with regulatory guidelines and agency standards.
• Interdisciplinary Collaboration: Coordinate closely with interdisciplinary team members, including home health nurses and physicians, to ensure seamless patient care.
• Regulatory Compliance: Maintain strict compliance with all local, state, and federal home health regulations, safety protocols, and company policies.

Qualifications

Education & Licensure

• Education: Doctorate or Master's degree in Physical Therapy from an accredited program.
• Licensure: Active, unencumbered Pennsylvania state Physical Therapist (PT) license.

Experience & Skills

• Experience: 1 year of clinical physical therapy experience is preferred. Prior experience in a home health setting is highly preferred but not required.
• Clinical Skills: Strong clinical assessment, diagnostic, and communication skills.
• Autonomy: Proven ability to work independently, manage time efficiently, and navigate a field-based, flexible schedule.
• Logistics: Must have reliable transportation, a valid driver's license, and up-to-date auto insurance.
