Oasis Therapy Services is seeking a Physical Therapist to provide rehabilitation services in a skilled nursing facility in Burr Ridge, Illinois. Available Employment Option PRN EMR: Net Health Responsibilities Evaluate residents and develop individualized physical therapy treatment plans. Provide treatment focused on strength, balance, gait, transfers, mobility, pain management, and fall prevention. Monitor residents' progress and modify treatment plans as needed. Complete discharge planning and provide recommendations for continued care. Educate residents, families, and caregivers on exercises, safety, and assistive equipment. Supervise Physical Therapist Assistants when assigned. Complete accurate Net Health documentation and collaborate with the interdisciplinary care team. Qualifications Active and current Illinois Physical Therapist license. Graduate of an accredited Physical Therapy program. Current CPR certification. Strong communication, organization, and documentation skills. Geriatric, skilled nursing, or rehabilitation experience is preferred. Why Join Oasis?
