Infusion Nurse II

Job Description Summary An Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home or in an alternate infusion suite, ensuring patient safety and continuity of care under a physician’s plan. Job Responsibilities Initiate, develop, and implement intermediate nursing plans, evaluating patient progress toward goals; administer treatments, medications, and educate patients and families. Assess patient needs and physical status during each skilled visit; reassess and modify care plans in response to changes. Obtain and clarify physician orders, promptly inform physicians of significant changes, and provide written summaries. Provide training and mentorship to Infusion Nurse I’s to ensure safety and compliance. Teach patients and families patient-centered care approaches to achieve independence in prescribed therapy. Communicate effectively with clinical teams, medical providers, patients, and families; prepare and submit clinical documentation in real time. Coordinate discharge planning; prepare and report discharge summaries with patient instructions. Ensure compliance with agency policies, professional standards, regulations, and accreditation requirements. Optimize work efficiency using computers and automation technologies to validate orders and manage visit schedules. Respond promptly to patient requests, initiate emergency procedures when necessary. Accept accountability for professional practice, following established programs and policies. Adhere to confidentiality and HIPAA regulations. Provide information on financial obligations related to services and charges. Attend required trainings, continue education, and maintain competence; oversee paraprofessionals in the home and act as Supervisor when necessary. Participate in multidisciplinary conferences, precept, train, and mentor other nurses for orientation and supervisory activities. Participate in nursing department on-call responsibilities. Perform other duties as directed by supervisor. Additional Responsibilities (if applicable) Insert PICC/Midlines per physician order. Troubleshoot or clean obstructed central line catheters. Monitor PICC/Midline and educate on infection prevention and routine care. Supervisory Responsibilities No supervisory responsibilities; the role does not include hiring, scheduling, performance reviews, or related duties. Basic Education and Experience Active, unrestricted RN license in the state of practice. Minimum of 1 year of nursing experience. Current CPR certification required. Basic Qualifications Demonstrated competency in patient care standards and infusion skills applicable to agency programs. License to operate a motor vehicle in the state of practice, access to a vehicle for business travel, and proof of liability insurance. Basic computer skills and ability to document care effectively using nursing technology tools. Physical Demand Requirements Can lift up to 50 pounds routinely; up to 75 pounds occasionally; may exert up to 20 pounds of force occasionally or 10 pounds frequently. Walking, standing, crouching, kneeling, turning, balancing, stooping, reaching, grasping, pushing, pulling, lifting and carrying. Exposure to blood, body fluids, and other potentially infectious materials; possible hazardous substances and unfavorable home environments. Must wear closed-toe shoes, face mask, goggles, gown and/or gloves, and maintain communication devices for routine and emergency situations. Travel Requirements Willing to travel up to 100% of the time for business purposes. Preferred Qualifications & Interests Bachelor of Science in Nursing preferred. 1–2 years of previous infusion nursing experience preferred.
