Full-Time

Silver Stream Rehabilitation and Nursing Center is seeking a motivated and experienced Director of Admissions to lead our admissions process and help grow our short-term rehabilitation and long-term care census. The ideal candidate is a strong communicator who understands the healthcare referral process and enjoys building relationships with hospitals, physicians, social workers, and families.

Responsibilities:

• Oversee and coordinate the admissions process from referral through admission

• Develop and maintain strong relationships with hospitals, physicians, social workers, case managers, and other referral sources

• Review referrals and coordinate with the clinical team to determine appropriate admissions

• Respond promptly to inquiries and referral opportunities

• Conduct facility tours and communicate the benefits and services of the facility to prospective residents and families

• Work closely with the marketing, nursing, and social services teams to ensure smooth admissions

• Follow up with referral sources and maintain strong ongoing relationships

• Monitor admissions activity, referral trends, and census opportunities

• Ensure admissions are completed accurately and efficiently in accordance with facility policies and regulations

• Provide excellent customer service to residents, families, and referral partners

Qualifications:

• Previous experience in admissions, marketing, business development, or healthcare preferred

• Skilled nursing or rehabilitation experience strongly preferred

• Excellent communication and relationship-building skills

• Strong organizational and follow-up skills

• Self-motivated, professional, and results-oriented

• Ability to work effectively in a fast-paced environment

• Knowledge of the local healthcare market and referral network a plus
