Key Responsibilities Deliver skilled nursing care per the Plan of Care during Medicare home health visits Accurately document visits in the EMR, including OASIS assessments Communicate changes in patient condition promptly with the interdisciplinary care team Provide clinical services within scope of practice, including: Medication administration Wound care Infusion therapy Catheter care Safe transferring and positioning of patients Collaborate with Nurse Supervisors and Managers of Clinical Practice Demonstrate professionalism, empathy, attention to detail, and strong clinical judgment Required Qualifications Active Florida Registered Nurse (RN) license Medicare home health experience required OASIS experience required Wound care experience required Proficiency with EMR systems (WellSky Kinnser preferred) Minimum 1 year of home health experience Strong communication, teamwork, and patient-focused skills Experience Details 1 year: OASIS (required) Alzheimer CEU approved by DOEA & HIV/AIDS CEUs (preferred) 1 year: CPR card (required) 1 year: Home Health Nurse (required) Why Join Activa Home Health?
