Outpatient Physical Therapist

The Outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans in accordance with patient needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for those assigned patients. The therapist ensures the design of functional treatment programs that facilitate effective return to community activities and serves as an educator for patients, families, students, coworkers, and self.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals aimed at progressing patients towards discharge.
• Document all medical records clearly, concisely, accurately, and in a timely manner, including goal writing and functional quality indicator assessments.
• Ensure care is provided under signed, valid physician orders and within insurance approval.
• Participate in the student program by providing a positive learning environment and supervision.
• Actively create an individualized professional development plan and pursue continuing education opportunities.
• Maintain professional licensure in good standing and adhere to the discipline-specific professional code of conduct.
• Contribute to organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ Year of outpatient clinical experience.
• CPR Certification.
• Active licensure for the state where work will be performed.
• Previous assignments with the agency are highly desired.

Schedule

This position operates on a 5x8.0 schedule.

Assignment Duration

This is a 13-week contract position.
