• Conduct medication reconciliation to support a seamless continuum of care for patients transitioning from hospital to home when applicable.
• Deliver outstanding customer service with a strong emphasis on clinical care.
• Provide patient consultations about prescribed medications, covering dosage, frequency, special requirements, and potential drug interactions.

• Active Illinois Pharmacist License.
