Guaranteed Hours: 40 hours/week

About the Role

We are hiring an experienced Physical Therapist for a 13-week contract in Petaluma, CA. This role is in a Skilled Nursing Facility setting and is a great opportunity for a PT who enjoys helping patients improve mobility, restore function, and regain independence.

Responsibilities

Evaluate patients' physical abilities and develop personalized treatment plans
Provide therapy to improve strength, mobility, balance, and functional ability
Educate patients and families on exercises, safe movement, and home strategies
Monitor patient progress and adjust treatment plans as needed
Collaborate with nurses, physicians, therapists, and interdisciplinary team members
Maintain accurate and timely documentation of assessments, progress, and treatment plans
Ensure care plans follow facility standards and therapy best practices

Requirements

Active California Physical Therapist license required
Minimum 1 year of Physical Therapy experience required
SNF or similar healthcare setting experience preferred
Bachelor's, Master's, or Doctoral degree in Physical Therapy required
Knowledge of physical therapy techniques, modalities, and adaptive equipment
PointClickCare experience preferred
Strong communication, documentation, and patient care skills
