On Call/Weekend Requirement: 7 days on 7 days off

Coverage Areas: 65 radial miles from office, Sedona, rimrock, camp verde, Clarkdale, cottonwood, Cornville.

Weekly Hour Guarantee: 40 hr

Productivity Expectations (points): 25

Will Milage be reimbursed between visits- yes, from first patient to last patient? N
