Collaborate with pharmacists and care teams to resolve discrepancies in medication lists.Patient Education and Support: Educate patients on medication use, side effects, and adherence strategies tailored to their needs. Address patient inquiries or connect them with the appropriate provider for further support.Medication Adherence Monitoring: Use EHR systems and other tools to track adherence to prescribed regimens. Proactively follow up with patients at risk of non-adherence and communicate concerns to care teams.Care Coordination: Assist with transitions of care, ensuring seamless medication management during discharges or care setting changes. Coordinate medication refills, prior authorizations, and delivery services to keep patients on track.Data Entry and Documentation: Maintain accurate, up-to-date patient records in the EHR system. Document patient interactions, interventions, and communications to support continuity of care.Support for Healthcare Teams: Collaborate with pharmacists to identify potential drug interactions or contraindications. Provide data and insights to enhance care plans and improve patient outcomes.Compliance and Quality Assurance: Ensure adherence to HIPAA, state pharmacy laws, and all relevant regulations.
