HF Care Navigator & Transitions Specialist

The Ohio State University Wexner Medical Center seeks a Heart Failure Program Care Navigation Liaison to coordinate post-acute care and reduce readmissions. You will serve as a trusted navigator across the care continuum, linking the medical team with post-acute partners and families to ensure smooth transitions. The role requires an RN with a BSN (Master’s preferred) and 5+ years of clinical experience.
