Job Summary SummaryClinical Social Work/Care Coordination 40 hour/week position divided between direct billable mental health and care coordination services. 3 onsite days as follows: Monday- Spaulding Salem Tuesday- Spaulding Boston Thursday- Spaulding Lexington Work directly with pediatric patients (and their families) in the outpatient rehabilitation setting who are injured or disabled, ill, or newly diagnosed with a pediatric condition and who may experience life altering changes in income, health insurance coverage, physical access to their environment, specialized transportation, and caregiver support to maximize independent function in the community. Collaborate across the continuum of care and with the interdisciplinary rehabilitation teams, community providers, and community agencies to promote the highest level of independent function and an active, productive, and fulfilling life. Direct and assist patients and their families in the application process for federal, state, and private resources and provide guidance to specialized community resources. Additionally, the ideal candidate will be experienced in assessing and providing therapy to patients with diverse clinical conditions including anxiety, depression, OCD, ADHD, Autism, Attachment, trauma (PTSD), developmental delay and often within the context of complex family systems and/or medical compromise. • Provide consultation to outpatient therapist as needed

Qualifications: Highly developed communications and interpersonal skills, working with diverse population. Work independently, be self-directed and flexible, and contribute as a member of a team. High level of service delivery. Demonstrate initiative with ability to prioritize workload, meet deadlines and adapt to changing situations. General working knowledge of available community resources. Ability to complete documentation in adherence to department and/or payer standards. Education/Degree Required: Master's degree from an accredited school or Master's degree in mental health counseling Licensure Required: Current, unrestricted license to practice in the state of Massachusetts: LMHC or LICSW Required: Access to reliable transportation to travel to pediatric outpatient sites across the Spaulding Network as necessary. Computer proficiency required. Microsoft office applications preferred with ability to learn new software. This position is grant funded.

Integration of prior clinical experience into current practice. Demonstrates proficiency to formulate assessment of risks, crisis intervention, psychosocial assessments, treatment plans, and the documentation of both in a timely manner. Demonstrates sound clinical judgment and innovation in advocating for clients while maintaining professional boundaries. Demonstrates proficient working knowledge of internal and external resources and refers appropriately. Provides a range of evidence-based interventions. Ability to advocate, coordinate, and continuously communicate with treatment plans with patients, family and the interdisciplinary team. Incorporates social sensitivity. If appropriate, utilizes supervision and consultation regularly and appropriately. Actively seeks assistance with complex case s and situations in a timely manner. Engages in quality improvement projects, uses data to measure progress. Facilitate skill-based groups. Knowledge, understanding and ability to negotiate and work in a complex organization. Demonstrate capacity to effectively communicate findings with a broader audience.
