Hospice Social Worker

Hospice Social Worker We are seeking a dedicated and compassionate Hospice Social Worker to join our team at Hospice of Western & Central Massachusetts in Worcester County. As a part-time Social Worker, you will travel to patients' homes and provide essential social services to meet their emotional and social needs. This is a unique opportunity to make a meaningful difference in the lives of patients and their families. This is an on-the-road position that requires the candidate to travel and have a reliable and insured vehicle. Weekly pay ranging from $41 to $44 an hour. LICSW or LCSW required. 24 hours a week. Responsibilities Assess the psychosocial status of patients and families/caregivers and communicate findings to the interdisciplinary team Carry out social evaluations and plan interventions based on evaluation findings Counsel patients and family/caregivers as needed and provide crisis intervention when necessary Educate patients and families on advanced directives and practical needs Provide information and referral services for patients and families/caregivers Serve as a liaison between patients and community agencies Maintain clinical records and participate in quality assessment performance improvement teams Requirements Master's in Social Work required Massachusetts LICSW or LCSW required Experience in healthcare and hospice preferred Ability to read, write, speak, and understand English Must be a licensed driver with an insured automobile Possess the ability to make independent decisions and deal tactfully with personnel, residents, family members, and the general public Benefits Competitive and Weekly Pay Holiday Pay for Hourly and Salaried Employees Overtime Pay for Hourly Employees Career Advancement Opportunities Exclusive Employer Discount Program Available for Eligible Team Members Employer Paid Life Insurance 401(k) with Employer Match Vacation and Personal Time Health, Dental, and Vision Insurance We are an equal opportunity employer that values diversity at all levels.
