Requirements

• Licensed Occupational Therapist in the state
• Current BLS/BCLS Certification
• Minimum of 2 years of experience as an Occupational Therapist
• Valid driver's license and appropriate auto insurance
• Basic understanding of insurance requirements in Home Health

Responsibilities

• Evaluate and establish a plan of care for patients in the home environment
• Provide direct patient care based on established care plans and physician orders
• Document skilled services and relevant communications in the medical record
• Participate in multidisciplinary conferences and communicate with care team members
• Supervise Occupational Therapy Assistants and CNAs as required
• Maintain licensure and participate in continuing education
