Job Description:

We are seeking a reliable and compassionate PRN Physical Therapist to provide one-on-one care to patients in their homes throughout Frisco and Carrollton. The ideal candidate is self-motivated, experienced in home health, and eager to help patients reach their full potential.

Key Responsibilities:

• Evaluate, develop, and implement individualized treatment plans.

• Deliver direct physical therapy services to patients in their homes.

• Maintain timely and accurate documentation in compliance with Joint Commission standards.

• Coordinate care with the Director of Nursing.

• Manage a flexible part-time schedule and attend all appointments punctually.

• Supervise and support a growing team of Physical Therapy Assistants (PTAs).

Qualifications:

• Degree from an accredited Physical Therapy program.

• Current Texas state licensure as a Physical Therapist.

• At least 1 year of experience as a PT (home health experience preferred).

• Reliable transportation and ability to travel between patient homes.
