Help ensure patients receive the compassionate care they need-when they need it most. At Visiting Nurse Service and Hospice of Suffolk (VNSHS), our Home Health Aide team plays a vital role in supporting patients throughout the community. As a Home Health Aide Scheduler, you'll serve as the central coordination hub for aide services, ensuring seamless communication, efficient scheduling, and exceptional patient support.

Your Impact:
The Home Health Aide Scheduler is responsible for coordinating and managing aide services for home care and hospice patients across Suffolk County. This role supports continuity of care by scheduling qualified aides, resolving coverage needs, and maintaining strong communication between patients, families, clinical staff, and contract agencies.

You Will:
Scheduling & Coordination
Coordinate and schedule home health aide services for newly referred patients within 24 hours
Build and maintain daily, weekly, and monthly aide schedules
Match aides to patients based on skills, location, availability, and patient needs
Resolve scheduling conflicts, call-outs, and coverage gaps in a timely manner
Monitor visit completion and follow up on missed or delayed visits

Communication & Support
Serve as a liaison between aides, nurses, therapists, patients, families, and contract agencies
Communicate schedule changes and service updates professionally and compassionately
Respond to incoming calls and messages promptly and efficiently
Escalate patient care or safety concerns to appropriate clinical leadership

Documentation & Compliance
Maintain accurate scheduling and visit records in the electronic health record
Track aide attendance, authorizations, and scheduling compliance
Support HIPAA compliance and confidentiality standards
Assist with audit readiness and required documentation processes

Team & Quality Support
Support onboarding and orientation of newly hired aides
Assist with monitoring aide compliance and attendance
Help identify workflow improvements and support quality initiatives
Promote a positive, team-oriented work environment

What You Bring:
Minimum 2 years of scheduling, staffing coordination, or healthcare administrative experience
Strong computer skills, including Microsoft Office (Word, Excel, Outlook)
Excellent communication, organizational, and multitasking abilities
Ability to work effectively in a fast-paced environment
Strong problem-solving skills and attention to detail
Ability to handle confidential information professionally and maintain HIPAA compliance

Preferred Qualifications:
Associate's or Bachelor's degree in Healthcare Administration, Business, or related field
Knowledge of home care regulations and insurance authorization processes
Experience with home health scheduling platforms preferred
Bilingual skills are a plus
