NEEDED: Pharmacists LOCATIONS:
- Salt Lake City, UT 84116
- Gunnison, UT 84634
- Must maintain all licenses required to lawfully perform the Pharmacist services and ensure they remain current.
- Maintain current, active, and unrestricted licenses in the State of Utah applicable to pharmacists
- Must submit proof of board certification or board eligibility in the Pharmacia field.
- Maintain a valid Controlled Substance License and Drug Enforcement Administration certification with no disciplinary actions.
- Prepare medications according to prescriptions written by authorized medical practitioners;
- Provide drug information and consultation to physicians, nurses, patients, and other clinical staff;
- Order and maintain a secure inventory of drugs, chemicals, and controlled substances;
- Fill and refill prescriptions as authorized;
- Prepare reports on medication and drug usage patterns; and
- Perform other pharmaceutical tasks assigned by DHHS staff.
