Registered Nurse (RN) – Home Health Care | Flexible Schedules | Home & Facility-Based Care

We are seeking a skilled Registered Nurse (RN) with Trach & Vent experience to provide high-quality, specialized patient care in home and facility-based settings. Schedule Options

• 6:00 AM – 3:00 PM | Mon–Fri (some weekends)
• 2:00 PM – 11:00 PM | Mon–Fri (some weekends)
• Flexible PT, FT & PRN opportunities available
• Shifts available: 4, 6, 8, and 12 hours based on client needs
• Short shifts (2 hours) available with premium pay

Key Responsibilities

• Provide skilled nursing care for pediatric, memory/dementia, and complex care patients
• Perform and manage Trach & Vent care, including airway and ventilator support
• Monitor patient condition and respond to clinical changes appropriately
• Assist with ADLs and patient mobility as needed
• Implement infection control and patient safety protocols
• Administer care per physician orders and clinical guidelines
• Maintain accurate, timely clinical documentation
• Communicate effectively with care team, patients, and families
• Ensure compliance with all safety, hygiene, and clinical standards
• Participate in ongoing training and skills development

Requirements

• Active Ohio RN license (unencumbered)
• Minimum 1 year clinical experience (home care or critical care preferred)
• Strong clinical judgment and independent decision-making skills
• Current BLS/CPR certification and TB clearance
• Reliable transportation & valid driver's license
• Ability to lift up to 50 lbs and assist with patient mobility
• Up-to-date vaccinations (COVID-19, Flu, Hep B)
