Registered Nurse - Home Health

Job Description

Job Summary:

Provides comprehensive home healthcare services to patients in their residences, ensuring safe and effective nursing care. Manages a patient caseload by performing assessments, implementing care plans, and collaborating with interdisciplinary teams to achieve optimal health outcomes.

Responsibilities:

• Conducts initial and ongoing assessments of patient needs, documenting findings accurately.
• Develops and updates individualized care plans based on patient assessments and goals.
• Educates patients and families on health management, medication usage, and preventive practices.
• Coordinates care with other healthcare professionals to provide holistic support.
• Performs direct nursing care interventions and ensures patient safety throughout services.
• Documents all patient interactions, treatments, and responses promptly and legibly per established policies.

Qualifications:

• Hold a current Registered Nurse (RN) licensure in the state of practice.
• Possess a Bachelor's Degree in Nursing (BSN) preferred.
• Demonstrate at least two years of recent experience in home health or acute care.
• Maintain valid CPR/BLS certification from an approved provider.
• Possess a valid driver's license, current automobile insurance, and reliable transportation.
• Be at least 21 years of age or older.
